A Clinical Study of Traumatic Abdominal Injuries

Abstract
This report is consist of review of 252 patients who admitted and operated on to surgical department of Daihan hospital during the past 4 years, from March 1987 to February 1991.
@ES We obtained following results.
@EN 1) The sex distribution was 206 males(81.7%) and 46 females(18.3%) with ratio 4.5 : 1, and the age distribution revealed a high incidence in third, fourth and fifth decades(74.6%).
2) The seasonal distribution showed the lower incidence in winter(15.5%).
3) Among the 252 cases, 218 cases(86.5%) were blunt injuries and 34 cases(13.5%) were penetrating injuries. The csuses of blunt trauma were traffic accident(149 cases, 59.1%), Violence(38 cases, 15.1%), Fall down(15 cases, 6.0%) industrial
accident(9
cases, 3.6%) and the causes of penetrating trauma were stab injury(33 cases, 13.1%) and gunshot(1 cases 0.4%).
4) The clinical manifestation were abdominal pain(96.4%), decreased bowel sound(81.7%), abdominal tenderness(79.6%), rigidity(78.6%), adbminal distension(62.3%) in order of frequency.
5) Shock status on admission(below 60 mmHg in systolic) were 16 cases with high mortality (37.5%).
6) The abdominal paracentesis showed positive in 109 cases(63.4%) and suspicious in 28 cases (16.3%) among 172 cases.
7) The peritoneal lavage was valuabale in diagnosis, positive finding was 29 cases(93.5%) among 31 cases.
8) Among 252 cases of X-ray checking the abdomen, free air was present in 27 cases(10.7%).
9) The injured organs were small intestine(38.1%), spleen(21%), liver(17.5%), mesentery(15.1%) in order to decreasing frequency.
10) The method of operative measurement were variable.
11) The multiple intraabdominal organ injury increased mortality.
12) The commonly associated extraabdominal injury were chest trauma(32.2%), skin laceration(24.2%), extremity fracture(16.7%).
13) The incidence of postoperative complications were 25.8% and frequent complications were wound infection(8.7%) and pulmonary complications(7.1%).
14) The causes of death were hypovoemic shock(4 cases), sepsis(3 cases), respiratory insufficiency(2 cases), head injury(2 cases) and renal failure(2 cases).
15) Overall mortality rate was 5.2%.
